Ilizarov Fixation System

Description

The Ilizarov Fixation System is an advanced orthopedic surgical technique designed to address a wide range of bone-related issues. Developed by Dr. Gavriil Abramovich Ilizarov, this system is utilized for multiple clinical purposes:

1. Bone Lengthening and Deformity Correction

The Ilizarov apparatus is used to lengthen or reshape damaged bones in the arms or legs. It is especially beneficial in the treatment of complex fractures, open bone injuries, and infected non-unions that cannot be resolved through conventional surgical methods.

2. Limb Salvage Technique

In cases of non-healing fractures, the Ilizarov system provides a limb-preserving surgical solution. By stimulating bone healing and regeneration, it enables the treatment of osteopathic non-unions without the need for amputation.

3. Distraction Osteogenesis

While experimenting with the adjustable rod frame of the external fixator, Dr. Ilizarov discovered the principle of distraction osteogenesis. By gradually expanding the frame, he observed the formation of a fibrocartilage callus at the fracture site, which promoted new bone growth over time.

4. Fixation Mechanics

The Ilizarov device consists of aluminum rings anchored to healthy bone using Kirschner wires and pins. Additional rings and threaded rods are connected with adjustable nuts to immobilize the affected limb. This circular frame provides controlled tension mechanics, facilitating both distraction osteogenesis and early weight-bearing during treatment.

5. Applications of the Ilizarov Circular Fixator

The Ilizarov system can:

  • Correct angular deformities in the legs

  • Resolve limb length discrepancies

  • Treat non-unions and malunions of osteopathic origin

  • Address structural defects in long bones by transporting bone segments and stimulating regeneration
